Mangalore: First Ever Cardiac Procedure Performed using FFR Technology at AJ Hospital

Mangalore, Dec 14: A 69 year old Swiss citizen recently underwent Coronary Angioplasty here for the main vessel (LAD), which showed borderline lesion of 60-70% on coronary angiography.

This procedure was done with the help of a new technology called Fractional Flow Reserve (FFR), by Dr BV Manjunath, Chief Cardiologist, AJ Hospital and Research Centre, Mangalore.

FFR is used to determine the significance of borderline Coronary Artery Disease to decide whether any procedure like angioplasty is required. This is the most accepted, scientific way of doing a procedure in the world, where the FFR calculates the gradient across of the stenotic lesion accurately, thus determining exactly its nature and management.

After this technology is used, unnecessary procedures like angioplasty or CABG (Bypass Surgery) for those borderline lesions can be avoided. At the same time, those lesions which look borderline by angio may be significant enough to cause ischemia in those area, which are missed requiring to undergo angioplasty. It also helps to identify and suggest to the cardiac surgeons regarding which vessels to be grafted during bypass surgery.

    FFR has certain advantages over other techniques to evaluate narrowed coronary arteries, such as coronary angiography, intravascular ultrasound or CT coronarography. For example, FFR takes into account collateral flow, which can render an anatomical blockage functionally unimportant. Also, standard angiography can underestimate or overestimate narrowing, because it only visualizes contrast inside a vessel.
    Other techniques can also provide information which FFR cannot. Intravascular ultrasound, for example, can provide information on plaque vulnerability, whereas FFR measures are only determined by plaque thickness.
    FFR allows real-time estimation of the effects of a narrowed vessel, and allows for simultaneous treatment with balloon dilatation and stenting. On the other hand, FFR is an invasive procedure for which non-invasive (less drastic) alternatives exist, such as cardiac stress testing. In this test, physical exercise or intravenous medication is used to increase the workload and oxygen demand of the heart muscle, and ischemia is detected using ECG changes or nuclear imaging.
