Udupi, Oct 31: A team of Doctors trained in Urogynecology, Pelvic floor Medicine & Reconstructive Surgery of Kasturba Hospital, Manipal performed a Tensionfree Vaginal Tape (TVT) by Retropubic Approach recently.
A 50-year-old female, who was suffering from severe SUI for more than 5 years. She appeared to be depressed and extremely anxious because of her condition and also frustrated because of multiple back and forth cross referrals from Gynaecologists to Urologists to Physiotherapist without any significant improvement in her suffering. On two months follow up visit, the patient is very satisfied by the procedure. According to the patient and her family this procedure has really made a difference in her quality of life. “She is much more confident, happy and social for last two months,” the patient’s husband said with a smile on his face. She is determined to educate other women in the community that – Urinary leakage is not normal and can be effectively managed with minimally invasive surgery.
Our Urogynecology surgical team that performed the procedures comprised of Dr Deeksha Pandey assisted by Dr Shripad Hebbar, Dr Richa Chouksey and Dr Swati Kanchan. Dr Deeksha has been trained in Germany for performing these challenging surgical procedures which are minimally invasive in nature and have great impact on patients’ quality life. In KH Manipal to provide holistic care under one roof to women having Urogynecological (Incontinence and Prolapse) issues, they have initiated an ‘Integrated Clinic’ that comprises of Urogynecologists, Physiotherapist and Psychologists.
This procedure is not a new one at the global platform. However, because of the technical challenges involved, despite providing an excellent outcome for the patient, it is not a friendly procedure among most of the surgeons. Stress Urinary Incontinence (SUI) that means involuntary loss of urine on coughing, sneezing or laughing - is a common problem. About one in three women suffer from SUI at some point in their lives. TVT is the standard of care for this condition. This minimally invasive surgical procedure can be performed by two approaches – 1) Retropubic (TVT) and 2) Trans-obturator (TVT-O). Trans-obturator approach is most commonly adopted in India mainly because of the ease of this procedure. Though, globally it has been accepted that Retropubic approach is superior in terms of efficacy. Retropubic approach also has less complications in long term. However, because of the scare of its per-operative complications and requirement of more expertise it is a less commonly performed procedure in India. These issues can be handled by adequate training, adoption of correct technique and use of an optimal Mesh- system.
