Pilot asleep, Jet flight from Mumbai plunges 5,000 ft; probe ordered


New Delhi, Aug 14 (PTI): In a major scare, a Jet Airways flight from Mumbai to Brussels, carrying around 280 passengers, plunged 5,000 feet while overflying Turkish airspace last week as the commander was asleep and the co- pilot busy with the iPad containing flight information.

Terming it a "serious incident", aviation regulator DGCA has suspended the two pilots and initiated a probe into the incident.

It has also set up a three-member team to review the airline's flight training programmes and facilities following the incident last Friday, official sources said, adding the team has been asked to submit their report by August 31.

DGCA has directed the airline to come forward with all related reports, as well as the records of the Digital Flight Data Recorder (DFDR) within this week, they said.

The incident occurred when the Boeing 777-300, operating flight 9W-228 from Mumbai to Brussels and onwards to Newark, plunged from 34,000 feet to 29,000 feet while traversing on the busy air route to Europe.

As the aircraft descended, the air traffic control at Ankara sent an emergency message to the aircraft asking the pilots why they had deviated from the assigned flight path and directed them to climb up to the designated height immediately.

Both pilots were summoned by DGCA yesterday for questioning.

While the commander said he was on "controlled rest", implying a short nap inside the cockpit which is allowed by flight operation procedures, the co-pilot told the regulator that she was working on her iPad or the electronic flight bag which has all aircraft documents loaded on to it, the sources said.

Immediately after the Ankara ATC message, the co-pilot woke up the commander who restored the height of the plane.

Confirming the incident, an airline spokesperson said Jet Airways has initiated an internal inquiry into the matter.

"The airline is also extending all co-operation in the matter to the DGCA by providing all necessary assistance for the inquiry. Safety is of paramount importance to Jet Airways as is also the welfare of our guests and crew and the airline will always take appropriate steps to ensure the same," the spokesperson said.

Following the incident, DGCA has constituted a team comprising officials from its Airworthiness, Air Safety and Flight Operations wings to audit Jet Airways' training procedures for pilots for an entire year.

The team would also look into the airline's training policies, including how it defined 'controlled rest' for a pilot, the sources said, adding that inspections would also be carried out of its training facilities.

The regulator would also probe why the airline or the pilots failed to report the incident to it, as the DGCA got to know of the incident only after an anonymous message, the sources said.

While both the pilots kept flying until they were grounded two days ago, they did not even file a flight safety report, which is a mandatory exercise to document any untoward incident during the course of a flight, they said.

The cockpit crew should have informed the airline which in turn should have apprised the Accident Investigation Board of the incident, the official sources added.

The DGCA would also enquire into whether there was any technical failure in the aircraft which led it to lose height and going for an 'uncontrolled descent' or was any switch accidentally pressed by any of the pilot, they said.

The Aircraft Accident Investigation Bureau has been asked to conduct a detailed investigation into the incident which could take 4-5 months.

Only after the Board gives its report, would it be possible to determine the quantum of penalty, if any, for the pilots, officials said.

    Precious lives of people mare in the hands of the pilots. It is indeed disheartening to learn that the pilots take snooze while on duty. Again for the co-pilot to be busy with the Ipad at a wrong time is inexcusable.

    I am glad that the airlines have taken serious view of this incident and that inquiries are in progress.

    BUT WHAT HAPPENED IN MANGALORE AVIATION CONTROL BEFORE 4 YEARS WHEN THE AIRCRAFT GONE OUT OF THE RUNWAY.
    WHAE THEY ARE DOING? THEY ARE THE ONE WHO IS GIVING GREEN SIGNAL FOR LANDING I MEAN HEIGHT,DISTANCE ETC.ETC.
    WE DIDNT HEARD ANY NOISE FROM COCKPIT RECORDER FROM MANGALORE AVIATION CONTROL AUTHORITIES IN THAT CRASH.

    Jet Airways had good customer satisfaction once but now that is sliding down considerably.

    While I was flying for the first time by Jet airways from Delhi to Bagdogra in West Bengal in 2006, the service and food were excellent. But next year, while I was flying from Delhi to Bagdogra, the food was great, but while I was sliding the back to take a nap, the rowdy passenger behind me complained and when I talked to the steward, he was supporting the rowdy. I told him to give me another seat, but that was ignored. That day, I felt ........towards the Jet airways due to the rude behavior of the steward so never flew JA again.

    One unfriendly employee can drive away a passenger for ever.

    Dear all, my recent experience with Jetkonnect flight.
    I boarded last flight from Mumbai at 2035 and was scheduled to land at 2205hrs.
    I was sitting at the window side and I was watching approach for landing. The flight descended for landing and I could see the runway clearly through the window and there was no rain. The flight was almost about to make contact the runway the Pilot aborted landing and straight away headed to Bangalore without making the second attempt for landing.All passengers were silent till Pilot announced as that due to bad weather the flight is diverted to Bangalore.In Bangalore we were made to sit inside the flight and again there the security persons boarded and asked every one to identify the baggage.later we took off from Bangalore and landed in Mangalore at 0020hrs. After landing in Mangalore I met one of my friend he said after 10 minutes of our flights diversion to Bangalore,the Air India flight landed with out any problem.

    Thank God for saving the people from
    big tragedy. All these pilots have lot of ego/head weight and they think
    what they do is right. Same thing happened with Airindia express crash in Mangalore.These pilots should be given extra training to be obedient and make them to understand that they are carrying so many lives in their flight. If they are on trial fligt all alone let them dive their flight and ful fill their smartness or let them join fighter-jets.Like some one said cancel their license.

    Jet thinks/treats Dubai Mangalore class is low class and does not treat them nicely . Soon they will repent if they start treating cutomers in this way. Looks like Air India staff has joined them now . Staff is not trained to treat internation passengers .Saw a jet airways staff asking a labour passenger "Do you have problem in hearing?" .Standard has fallen obviously

    @Sudhakar Poojary Dubai.,what or who is there to wake the helper in case he too sleeps.?what a luck that atleast the Turkey aircontrol were awake.otherwise many People had their flight for the last time.Of course we blame all who make failures.Afterall the Crew have Long journey to without break,specially pilots who had to be alert all time .but there are instances ,when Long journey had to made ,eyes Close automatically in a way of driving for Long ,and you sleep without your knowledge in a Sekunden sleep they call.and in this sekend,anything can happen,accident,or car slide to side.May be this was the reason.(I am not defending the Pilot,but such cases occur.)

Some days back I had seen one doctor in this forum defending a doctor who left scissor in patients body and now that doctor is criticizing the pilot. I assume this pilot might have been overworked. Flight is not like a bus or train where it can be stopped in the middle of the journey. Thorough investigation will find out the truth. But here people are already started judging. Hypocrites.

    We don't see the pilots, we don't know anything about them but simply by faith boarding the plane trusting to land at the final destination.

    These crashes-pilot' sleeps etc are getting too scary to fly!

    Once one flight of certain airlines from Singapore to Mumbai, flew over Mumbai and the pilots were alerted by the Mumbai terminal about the misguided flight, so the pilots woke up and returned to the airport. Whether bound?

    Jet airways is well known for miserly tactics. Crew is probably made to overwork.
    It is known fact that this Airlines takes extreme measures to save money. The aircrafts are old and dirty, none of the TV screens work, seats are broken, food is third grade, service is fourth grade. Do not even follow rules regarding compensation for lost baggage, use old buses for transport inside airport, overload the buses with 70 to 80 passengers.
    Now they have gone to the extent of using pilots without sleep.
    Just because we were used to bad service from Air India or Indian Airlines, many may not be aware of what is the standard of other airlines worldwide.

    Good to know nothing happened. I am travelling normally by other flights all over world.Recently switched couple of trips by Jet Airways international. I have very good experience by jet towards indian sector and they are very good indeed. By international flight standards Jet Airways need to improve its services in few of flights especially staff courtesy. Few of the staff not trained to cater to international standards for sure. Jet cannot treat all sectors same and they must treat customers with great care .If they want to gather more popularity they need to improve standards for sure in few flights

    This is the height of irresponsibility. The pilot was taking a nap while the co-pilot was busy browsing the tablet.

    This is not a good sign, But how could a flight plunge so many feet(if pilot is asleep) ? in cruise flight is fully automated with Auto pilot settings. And more over how come copilot has to wake the pilot up? the copilot can make the corrections if its an emergency??
