Bengaluru: I-T unearths doctor-medical centre nexus running into crores


Bengaluru, Dec 2 (PTI): Income-Tax officials have unearthed a major multi-layered nexus between medical centres and doctors and an undisclosed income of Rs 100 crore after searches at a few IVF clinics and diagnostic centres here, the department said on Saturday.

It claimed doctors were being paid "for referring medical tests". IT sleuths, the department said, have seized about Rs 1.4 crore in cash and about 3.5 kg of jewellery and bullion during its three-day action against two in-vitro fertilisation (IVF) centres and five diagnostic centres.

They also seized foreign currency and detected foreign bank accounts with "deposits running into crores of rupees".

While the labs searched have declared an undisclosed income of over Rs 100 crore, the amount of referral fee in case of a single lab is more than Rs 200 crore, it said in a statement.

The searches at the diagnostic centres, a senior official said, uncovered various methods by which doctors were being paid for referring medical tests.

"The commission varied from lab to lab but the median range of normal cut/commission for doctors is 35 per cent in case of MRI tests and 20 per cent in case of CT scan and other lab tests. These payments are found to be disguised as marketing expenses," the statement read.

The department said referral fee to doctors was paid in at least four ways, including fortnightly cash payment and advance cash payment.

In some cases, referral fee paid to doctors by cheque was shown in books as "professional fee".

As part of a deal, doctors were employed as in-house consultants. "However, neither they come to diagnostic centres nor see patients or write reports. This payment is a disguised referral fee," the statement read.

The department claimed referral fee was paid to doctors by cheques under "revenue sharing agreements".

"Some of the labs employ commission agents whose job is to distribute money to doctors in envelopes. These agents insert a small chit in envelopes while giving them to doctors which contains the details -- names of patients referred by the doctor, doctor's name, tests done, amounts billed, cut/commission amount to be given to the doctor and the date on which the test was done," it said.

    Has any doctor responded so far? If our government can pass a law that jils husband's giving triple talaq .why can't we pass a law making all cash only transactjons illegal?

    We the citizens demand such a law to be passed. I do not think any doctor or the IMA will have guys to oppose it!

    Drug companies take doctors and their families on holidays abroad and also they get commission for admitting patients into private nursing homes. Unnecessary surgery is performed and patients are chrged for it. I pity the poor people caught in this medical mafia nexus.

    Every city has Doctors and other professionals who deal with huge amount of cash. In Mangalore there are Doctors who take very high consultation fee in cash only. No receipt. No card swipe machine (POS). They have full time staff to take appointments, but say that they can't afford a staff to collect cash and issue receipt. The common man insists on bills everywhere else but not here because of the respect for the doctor. The doctor takes advantage of this reverence.
    Regarding MRI & CT scan, there are Doctors who don't just refer you to particular diagnostic centres, but they fill up that particular centre's form and give you, leaving you no choice but to go there.

    Sad to say, scams are rampant in Indian health sector. The prominent scams are

    1. Duplicate medicine sold by pharmacies.
    2. Doctors prescribing certain medicines which are either in-effective or fake generating easy money for pharma companies and doctors.

    There may be many many more, but these are the once i have noticed. Strong government regulation is required to curb these scams.

    2. Recently in Delhi, an infant declared dead by a doctor in an upscale hospital was found alive. Any accountability and punishment for this criminal negligence?

    3. I was vociferous in my comments about lab- commission (cut) paid to these doctors. What punishment is due for these under Indian penal code?

    4. Any Indian citizen who is wilfully evading tax and defrauding Indian government should be held accountable. There are more than enough doctors to take their place.
    5. I am sure if there were sting operations, many doctors in Mangalore can be indicted and brought to justice. Patriotism starts at grass root level.

    Side effects of very high capitation fees.

    How many genuine medical aspirants lose their battle when it comes to crores of capitation fees and settle for other professions without any passion but only as a means of survival!!
    How many genuine low income aspirants lose their battle because of reservations and lose chance!! Those who bag seats under reservation seldom do not reach where they want to be as the same has come without any goal or passion or efforts. Its not that all candidates under reservation are bad but this has to be identified precisely and then to decide, so that deserving aspirants do not lose their passion for this noble profession. If this happens, then there won't be any unethical practices to cover up their huge investment that has gone on the pretext of capitation.
    It is always easy to punish some scapegoats, but none in power are ready to give remedy. Do we all agree "prevention is better than cure."

    Thanks Jossey Sir, But nothing is going to change soon, Because first, People need to change their mentality, I mean the way of thinking. We want our house to be clean but we throw garbage out of our compound. We don't want any one to look at our sisters but we stair at every female walk pass by us. We complaint about people accepting bribe but we pay them to get our work done and Finally we elect people who are corrupt and not capable to do the job as our leaders. We need to looks it to the root cause and work on it. This will take over 20 years in India. I hope our new generation will adopt and understand this soon So, we will have better living environment and respect for each others irrespective of their cast, gender or the age.

    Scams are not new, they were there when your beloved Congees were in power, and you were not fed up then. Scams have always been there. People bribe doctors and many other professionals, especially tax officials, and even judges, lawyers and court officials; people just want to get out of hassle or save themselves. India is not alone in doctors getting "cuts" from drug and pharmaceutical companies - they do it everywhere, even in Canada. This is the society we live in.
    The best way to deal with these ill-gotten gains is to tax them and use the Prevention of Money Laundering Act (which, by the way, was brought in by the Congress Government) to disgorge all the gains made out of such under-the-table dealings, in any form, including gold and valuables, and properties. That Act is a potent weapon but, sadly, used only against drug peddlers.

    Sad that the so called noble profession has gone to dogs due to greed by payment seat qualified professionals (most) !!! Poor patients go to doctors, do the tests prescribed by them blindly trusting them even if many can't afford by borrowing. See how commission agents in this network illegally making money.

    Even central government is not bringing this biggest black money generator under GST net even by charging nominal 1% nominal rate. Reason ? Best known because of kickbacks enjoyed by many in the loop.

    Eye care chains, dental/cosmetic clinics, x-ray, scan centres, hospital, pharma companies giving kickbacks to doctors to refer their products all needs to raided & their call records tracked for illegal business practices.

    Where is digital india ? Where is IT department ? Where is government ?

    Introduce measures to track majority cash dealing & tax avoiding sectors like so called professional consultants, meter interest thugs, money lenders, caterers, real estate dealings etc.

    Introduce "No Bill - No Pay" strict policy to entire India if you can't control majority cash transactions as these guys don't accept cheque or digital payments in most cases.

    @George mangalore.when you do this ,be sure that their lawyers will file a case on you for spoiling doctors name. Best is modi should bringout a law that all these doctors demand money cash without a reciept, must giveout reciept or sameway as pay ment as you pay in any grossery market .as usual in Inda at present.
